Transcripts
The Ontario Student Transcript (OST) is the official record of a student’s academic achievement at the secondary school level in the province of Ontario. The OST is retained by the last secondary school that you attended. If the last secondary school you attended resided within the Catholic District School Board of Eastern Ontario and you require your OST, please refer to the applicable process listed below to request your transcript.
Current CDSBEO Students:
If you are a current CDSBEO secondary school student, or the parent/guardian of a secondary school student who is not an adult (17 years of age or younger), please contact the student’s secondary school Student Services Department to request student records.
Former CDSBEO Students:
Students who have graduated or retired from school shall request an OST by completing our Request for Transcript Form.
- In Person:
Submit the completed form and the applicable non-refundable transcript fee (cash or money order) to the last secondary school you attended within the board. You will be required to show proof of identification when picking up your OST. If your transcript request form is being submitted by someone other than yourself, you must also attach a photocopy of your proof of identification to the completed request for transcript form.
- By mail:
Please mail the completed form along with the non-refundable transcript fee (money order payable to last secondary school you attended) and a photocopy of your proof of identity directly to the last secondary school you attended within the board. To view a list of CDSBEO schools please visit:

Transcripts Required for Post-Secondary Application in Ontario (OCAS/OUAC)

Current students planning to attend college or university in Ontario will receive information from their school’s Student Services Department on how to apply via a Post-Secondary Application Service (OCAS and/or OUAC). Transcript information is uploaded electronically to OCAS and/or OUAC by your school on your behalf.
If you are a former student of the CDSBEO applying online to an Ontario university or college through OCAS and/or OUAC, your transcript must be requested as part of the online registration process.
Fees
The fee is $20.00 for the first copy of the OST, and $5.00 for each additional copy ordered on one request form (non-refundable) payable to the last secondary school you attended.

Valid Proof of Identification
Valid proof of identification will be required for those picking up records in-person. For those requesting electronic copies of their records, a scan or photograph of a valid ID will be required before any records can be released.
Processing Time
Processing time is approximately 2 weeks, not including mail delivery time. Processing time may be longer during peak periods. Please note, visiting our office in person does not guarantee you will immediately receive your records.
Important Information
- We cannot release any student information over the phone.
- We cannot process a transcript request if the last secondary school you attended was not with the Catholic District School Board of Eastern Ontario.
- Student records are confidential. If the student is 18 years of age or over, records can only be issued to a third party with written consent from the student.
